Confirming neuroleptic-resistant schizophrenia

چکیده
standard’ treatment for treatment-resistant schizophrenia, after failure of two antipsychotics at adequate doses for an adequate duration of time. However, even after 1 year, a number of patients fail to respond to clozapine alone. We question the term ‘treatment-resistant’, which implies that little further can be done and generates therapeutic nihilism, and instead suggest the term ‘neuroleptic-resistant schizophrenia’ as a more positive alternative. In this editorial we suggest a number of treatment options for patients resistant to clozapine monotherapy, and hope to generate a fresh and positive approach.

One more look at propranolol for the treatment of refractory schizophrenia.
Propranolol, a beta-adrenergic blocking agent, has been proposed previously as potentially useful in the treatment of certain otherwise treatment-unresponsive psychotic patients. This article reviews the published clinical trials of the efficacy of propranolol in schizophrenia to characterize those patients in whom it might be helpful and for whom future clinical trials should be designed. Desp...

Aripiprazole, an atypical antipsychotic with a novel method of action, has only recently been awarded a license in the UK. We report our first patient to receive this drug, who had treatment-resistant schizophrenia and developed neuroleptic malignant syndrome (NMS) with aripiprazole. To our knowledge, this is the first published case report involving aripiprazole and NMS in a potentially fatal ...

متن کاملNon Responsiveness to Ect -- an Additional Pre-requisite for Clozapine Therapy
C. Andrade in his letter published in the Indian Journal of Psychiatry Vol 38, No.2 Page 117, has suggested that non responsiveness to ECT should also be considered an additional pre-requisite for patients who are to receive clozapine. For the reference of the readers of Indian Journal of Psychiatry, I would like to mention the views of some of the foreign authors on this issue. The clinical re...
